House Calls Are Back!

Our Primary Care at Home team provides house calls by our primary care providers, chronic care management, care coordination and daily health monitoring.

These services allow our team to detect issues early, so we can intervene early to prevent unnecessary trips to the hospital. With our Primary Care at Home practice as the center of your care, Freudenthal can also surround you with home health care, in-home personal care services and hospice care as needed.

We also coordinate and collaborate with a high-value network of your other providers such as specialist physicians, hospitals, pharmacies, etc.
