Customizable  Health Monitoring

Freudenthal Primary Care at Home provides customizable health monitoring for our patients. Our health monitoring services include wellness calls by our care coordinators (or optional automated calls), as well as a variety of remote patient monitoring devices that collect important health data as needed including:

The Remote Patient Monitoring (RPM) devices used by Freudenthal Primary Care at Home are reliable, easy-to-use and delivers data in real time to our care team. Unlike devices used by other local programs, our devices don't have to be linked to wi-fi, a computer, tablet or phone in order to send data to our team.

 

It's as easy as pushing a button or stepping on the scale. These easy to use devices require no set up on your part and are delivered to you by our team.
