Position Summary

The Director of Rehabilitation is responsible for the coordination, oversight, and quality of therapy services within our Home Health division. This role ensures compliance with Medicare regulations, supervises therapy staff, and promotes effective, patient-centered care.


This position may also serve as the Clinical Manager as outlined in Medicare Conditions of Participation.


What You’ll Do

Leadership & Operations

  • Coordinate and manage therapy services for home health patients

  • Review referrals and assign appropriate therapists based on patient needs

  • Provide supervision, guidance, and support to therapy staff

  • Assist with hiring, onboarding, and training of therapy personnel

  • Maintain accountability for productivity, utilization, and documentation standards

  • Participate in case conferences to ensure coordinated care

Clinical Oversight & Quality

  • Review and evaluate patient cases, documentation, and care plans

  • Assist therapists in setting goals and developing effective treatment plans

  • Conduct quarterly chart audits and implement quality improvement plans

  • Perform on-site supervisory visits as needed

  • Maintain department QA reports and address deficiencies
     

Compliance & Documentation

  • Ensure medical records meet all Medicare and regulatory requirements

Qualifications

  • Current state license as a Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP)

  • Graduate of an accredited therapy program

  • Minimum 2 years of management or supervisory experience (home health preferred)

  • Current CPR certification

  • Valid driver’s license, reliable transportation, and required insurance

  • Strong clinical judgment, organizational, and leadership skills

  • Excellent verbal and written communication abilities

We are seeking In-Home Caregivers for ALL Shifts. We need caregivers who can be flexible and manage multiple shifts with different clients. Being a caregiver with Freudenthal is a definite calling that is very rewarding. In-home caregivers build lasting relationships and give assistance to people who are sick, injured, physically disabled, or the elderly and fragile. Caregivers work in a home setting and help their clients with activities of daily living, such as bathing and bathroom functions, feeding, grooming, taking medication, and housework, to name a few.

Caregivers help clients make and keep appointments with doctors, provide or arrange transportation, and serve as a companion for their clients. Caregivers maintain open communication between families and administration staff regarding the client's medical and emotional condition. Caregivers also document and report any changes in the client's health status, as well as ensure client safety and well-be. They are our front line of care at Freudenthal.

  • Provides personal care services and companion services according to a written Plan of Care.

  • Every other weekend mandatory.

  • Provides personal care services and assists in activities of daily living (ADL) including, but not limited to, personal hygiene (bathing, showering, shaving, etc.), eating, dressing, and ambulation.

  • Performs general household duties essential to health care at home.

  • Gets approval from Executive Director to perform other services not listed above.

  • Observes, records, and reports any significant changes in the client's condition to the Executive Director.

  • Performs other related duties as directed by the Executive Director.

  • Other job duties may include, but are not limited to: Planning and preparing meals, shopping, providing transportation, reminding the client to take medications, delivering pre-setup medications, assisting with walking, letter writing and correspondence, taking out the garbage, changing linens, helping with clothing selection, maintaining client calendar, discussing current events, playing games, caring for house plants, participating in crafts, assisting with pet care, laundry and ironing, making beds, dropping off and picking up dry cleaning, overseeing home deliveries, supervising home maintenance, general housekeeping, monitoring food expirations, etc.

Requirements

  • Detail Oriented: Some clients have specific rules or schedules that must be minded or specific rules that must be followed.

  • People Skills: Caregivers work in a very personal way with their clients. Some will be in pain or very sensitive to their fragility. Caregivers must be sensitive and compassionate with clients.

  • Schedule Driven: Caregivers are schedule keepers. Caregivers have to be there to make sure clients get up on time, make sure medication is taken on schedule and clients get to appointments on time.

  • Responsibility: Caregivers are trusted to provide the best care they can to their clients. In many instances, a client's well being and life may depend on their caregiver. It is for that reason that caregivers must be able to be trusted and must constantly demonstrate that they are capable, and responsible people. Throughout the duration of employment with Freudenthal Home Health, LLC, the Personal Care Assistant must meet the full job description as stated. If at any time during employment, you do not meet the full job description; your status may be changed to inactive until a physician's release is turned into the office.

  • Must have: A valid driver’s license & proof of auto insurance.


Qualifications and Skills

  • Eighteen (18) years of age or older.

  • Previous experience is preferred* but not required. Previous agency or facility caregiving experience, that can be validated, and/or current “training” or “medication” certifications can increase your pay scale.

  • Ability to read, write, follow directions and perform duties with minimum supervision. Ability to show respect, kindness, and genuine concern 100% of the time toward clients, families, and coworkers. Provision of reliable transportation/insurance and a working telephone as required by the nature of the position. Requires at least two weekend availability per month.

The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Provides physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act. This may include, but will not be limited to:

  • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.

  • Assesses for muscle strength, mobility, gait, ROM-potential for rehab.

  • Directing physical therapy treatment.

  • Instructing patients and families/caregivers in the use and care of therapeutic appliances.

  • Determining priority needs for physical therapy.

  • Reporting to physician patient's reaction to treatment or changes in condition.

  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.

  • May train patient in the use of a prosthetic device.

  • Identifies patient and family/caregiver needs for other home health services and refers as necessary.

  • Prepares and submits clinical and progress summaries based on the attainment of goals.

  • Participates in discharge planning for patient.

  • Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.

  • Provides physical therapy consultation to home families/caregivers when indicated.

  • Provides in-service education programs for nursing organization personnel as needed.

  • Participates in peer consultation process.

  • Supervises physical therapy assistants according to organization policy and state regulations.

  • When therapy is the only skilled service, instructs, supervises and evaluates home health aide care every two (2) weeks.

POSITION QUALIFICATIONS

The speech-language pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.
 

  • Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

  • Identifies functional speech defects and establishes a plan of care to improve the patient's function.

  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

  • Recommends communication devices/aids as indicated.

  • Confers with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

  • As appropriate, participates in case conferences, inservices and other meetings required to ensure coordinated and comprehensive care.

  • Prepares and submits notes per organization policy.

  • Maintains clinical competency in speech-language pathology practice and theory.

  • Identifies patient and family/caregiver needs for other services and refers as appropriate.

  • Instructs patient and family/caregiver and other Organization health care personnel in the patient’s treatment regime as indicated.

  • May supervise home health aid in procedures directly related to the speech-language therapy plan of care.

  • Performs other duties as required to facilitate the delivery of speech-language pathology services.


Qualifications:

  • Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements:Is licensed as a speech-language pathologists by the State in which the individual furnishes such service; or If the State does not license speech-language pathologists:Must successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);

  • Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or related field; and

  • Successfully completed an approved national examination in SLP

  • A minimum of one (1) year of clinical experience in Speech-Language Pathology.
